- Name of school/business/organization
- Address of school/business/organization
- County
- Number of students/employee/church attendees
- Name of primary contact
- Phone number of primary contact
- Email of primary contact
Once we receive the information, we will contact the primary contact you list below for instructions on how to submit your closing.
Note: Please give us at least a week to process your request before contacting us again.
