JOHNSON COUNTY, Ind. (Jan. 4, 2016) – Equipment failure is blamed for a skydiving accident that killed a Martinsville woman.

Teresa “Terrie” Woods, 54, landed hard around 11:40 a.m. Friday near Franklin Flying Field and went to Johnson Memorial Hospital. Woods later succumbed to her injuries.

According to the Daily Journal, a skydiving instructor said Woods had a problem with her main parachute. She was able to get her reserve chute open. Woods was an experienced skydiver who’d made more than 1,300 jumps before the incident on New Year’s Day, the newspaper reported.

The FAA is investigating the accident.
